Make sure you are using the correct form version for your due date. Remember, the intended due date for your application determines the correct form version to use. If you are applying to a due date on or after January 25, 2022 you MUST use application forms with a FORMS-G Competition ID.

As you prepare your NIH grant application, keep these annotated form sets handy for key tips on filling out each section. These documents are a great visual resource for understanding many of the business rule checks we will run against your submitted application. Of course, you MUST follow the instructions in your funding opportunity announcement and application guide, but these documents are helpful for those times when you just need a quick reference.

NIH is transitioning to an updated set of application forms we refer to as FORMS-F. Use FORMS-F forms for grant application due dates on or after May 25, 2020 and FORMS-E for due dates on or before May 24, 2020.
March 3, 2020
Although the FORMS-F version of our grant application forms won’t be used until due dates on or after May 25, 2020, preparation for their use is well underway. Whether choosing between FORMS-E and FORMS-F application packages on non-parent FOAs or selecting the appropriate parent announcement, carefully select the correct version for your intended due date.
December 3, 2019
NIH will require the use of updated application forms and instructions (FORMS-F) for due dates on or after May 25, 2020. In the meantime, continue to use FORMS-E application packages for due dates on or before May 24, 2020 despite the expiration dates noted on each form.

You might be surprised to learn that we don’t have a generic set of forms posted on our website that you can use to submit to any NIH grant opportunity. Each funding opportunity announcement (FOA) includes the specific set of forms needed to apply to that program, so you need to find an FOA in order to access the application forms.
